We at The Mann Ear, Nose & Throat Clinic thank you for allowing us to provide your specialty care in ENT. Our goal over the past 20 years has been to create a visit that is not only helpful for your continued wellness, but also a pleasant and efficient experience for you and your family. With our Online New Patient Registration capabilities (which you may access to the right of this page), your medical information will be automatically transferred to our electronic medical records program, which will save significant time during your visit. If you do not wish to transmit your information over the internet, simply fill out the information as you normally would, but instead of hitting the "submit" button at the bottom of the page after filling out the form, print the information out at home and present it to the receptionist when you arrive.
Certain forms, by law, require an original written signature, and you will be asked to fill in a HIPAA and Patient Demographic Form by our receptionist when you alert her to your arrival. In order to expedite your visit, these forms may also be printed out in advance and signed, if you desire, by clicking on this link. By submitting both your medical information over the internet, and bringing the signed HIPAA and Patient Demographic documents to your first visit, our hope is that you may see your provider in a more expedient manner.
On the day of your appointment, please arrive 10-15 minutes earlier than the noted time so that we may confirm that you are fully prepared for your visit.
Please bring your insurance card, photo ID and a valid method of payment to each and every visit. Copayments, deductibles, fees and balances which are not covered by your insurance are collected when services are rendered. We accept personal checks, Visa, MasterCard and American Express. If you are scheduled for surgery, you are responsible for any deductible or co-insurance payments as required by your contract with your insurance company. In addition, any non-covered charges, such as cosmetic procedures, will be collected at the time of service. Bills from Mann ENT for surgical procedures do not include hospital charges, as we only bill for the physician component of the service. We will be happy to assist you in determining these charges prior to proceeding with your treatment.
Our office will file your insurance for you as a courtesy. Please note that your insurance company requires that we submit your social security number with your claim. If you do not wish to submit this information, your claim may be rejected by your insurance company, resulting in additional out of pocket expenses! For insurance plans with whom we participate, we will automatically file the claim. Please be advised that even if you do have insurance, some services (such as cosmetic procedures) may be considered non-covered or not medically necessary under Medicare and/or other medical insurance programs, and the charges will be your personal responsibility. If you do not have health insurance, please remember that all professional services rendered will be charged to you. The receptionist will give you a good faith estimate prior to your appointment of what these charges will likely be, but urgent or medically prudent treatment during your visit, as determined by your provider, may cause this estimate to be inaccurate.
In order to ensure a more efficient visit at Mann ENT, we have Pilar Berg, PA-C ready to help our physicians expedite your appointment. With her extensive experience, you may find that on some occasions she may be able to provide all of the services you require. Nonetheless, it is our policy that you may see your doctor at each and every visit, as you desire.
