Please read this page if your doctor has recommended surgery for you or your family member. We at The Mann Ear, Nose and Throat Clinic look forward to serving you. This sheet contains helpful information
regarding your surgery.

Medication Advisory
***Please do
not take aspirin, ibuprofen, anti-inflammatories, fish oil, flax seed oil,
Vitamin E, Plavix, Coumadin (Warfarin) and/or any blood-thinning medications for 3
weeks prior to, and for 3 weeks after your procedure, unless directed otherwise by your physician. If you have any questions about whether
your medication contains blood thinning agents please contact your pharmacist
or a clinical representative at Mann ENT.
HOSPITAL INFORMATION
You will receive
financial statements not only from our office but also from the facility at which your
surgery is performed. Please call
that facility PRIOR to having the surgery if you have any questions regarding
their billing policies. Our financial policies are available here. The phone
number for each facility is listed above. Please remember that the
facility will call you to confirm your arrival time to the hospital and what
time your surgery is expected to start. If you do not hear from the hospital by the afternoon prior to your
surgery, call our office for assistance. In general, you will be asked to arrive at the facility 2 hours before

FINANCIAL
INFORMATION
Surgery time slots are
valuable to both the physician and the patient. If you find it necessary to cancel or reschedule your
surgery, please do so at least 2 weeks prior to your surgery date. With less than one week's notice we will apply a rescheduling fee of $50.00 to your account that will be required to reserve your position on the schedule. This fee will be applied to any deductible or copay at the time of your procedure.
Benefit Verification and
Financial Obligations for Hospital Surgery and Office Procedure Patients
Benefit Verification
As a courtesy to our
surgery and procedure patients, your insurance benefits with your insurance carrier
will be verified prior to your surgery or procedure. You will be contacted, by
phone or letter, approximately two weeks prior to your surgery or procedure to
inform you of your financial obligation. If you desire to know your benefit information prior to two weeks or you
would just like to personally verify your benefits, we highly recommend and
welcome you to contact your insurance carrier for that information. We will supply you with the
corresponding surgery or procedure codes (CPT) and diagnosis codes (ICD-9-CM).
Financial Obligations
For surgery patients: It is
our practice policy for all deductibles, coinsurance amounts, co-pays and or
outstanding balances to be collected at check-in the day of your pre-op. For surgeries cosmetic in nature, no
financial arrangements can be made. Payment is expected two weeks prior to the surgery. Amounts stated are only as estimate of
your charges for Mann ENT Clinic. You will receive separate statements from the anesthesiologist,
operating room, hospital room, laboratory, radiology, assisting surgeon, etc.
For
office procedure patients: It is our
practice policy for all deductibles, coinsurance amounts, copays, and or
outstanding balances to be collected at check-in the day of your
procedure. For surgeries cosmetic
in nature, no financial arrangements can be made. Payment is expected two weeks prior to the procedure. Amounts stated are only an estimate of
your charges.
Verification Is Not A
Guarantee Of Payment
Verification of benefits
is not a guarantee of payment. Final determination is made upon receipt of the claim per your insurance
carrier. Your insurance coverage
is an agreement between you and your carrier. Therefore, your insurer has 30 days to pay or deny your
claim. If your insurer decides not
to pay your claim, you are responsible for all charges not covered by your
policy in addition to your deductibles and or coinsurance amounts stated in
your insurance contract.
