For new patient appointments, please call the office at (919) 859-4744. After having made an appointment, you may register with us online, beginning from this page, in order to save time once you reach the office.

Please read the disclaimer prior to proceeding, and select "I Accept" if you wish to proceed with online registration. You will then be directed to the secured registration page. By selecting the "submit" button at the bottom of the registration page, your medical information will be automatically transferred to our office, where it will be entered into an electronic medical records program.

If you do not wish to submit your information over our secure server, you may simply print out the page after filling in the appropriate information. Please bring this information with you to your initial visit.
